After completing this form, including required signatures, please return to the ESM graduate secretary in 327 Norris Hall (graduate students), or the ESM undergraduate secretary in 333-D Norris Hall (undergraduate students), for processing.

DISSERTATION PROPOSAL

Please notify the ESM graduate enrollment specialist in 327 Norris of your proposal date, time and place. Take this form with you to the proposal, get signatures of all committee members at the exam, then take this form to the graduate enrollment specialist in 327 Norris so that it may be placed in your file.
